Cancellation and Missed Appointment Policy
At Woodberry Family Practice, we understand that sometimes unforeseen circumstances may require you to cancel or reschedule your appointment. However, missed appointments or late cancellations prevent other patients from receiving the care they need.
Our Policy
- Cancellations: We kindly ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. This allows us to offer the time slot to another patient.
- Missed Appointments: If you miss three or more appointments without providing sufficient notice, a $30 fee will be applied. This fee must be paid before booking your next appointment.
- Late Arrivals: If you arrive late to your appointment, we may need to reschedule it to ensure we stay on time for other patients.
We appreciate your understanding and cooperation in helping us provide timely and quality care to all our patients.
Results and Reminders
Test Results:

- Test results will generally be available within 3-5 business days. Please note that our practice does not routinely contact patients for normal results. However, if a follow-up is necessary, our staff will reach out to you promptly.
- If you have not heard from us within 7 days and are concerned about your results, please contact the practice to follow up.
Appointment Reminders
- For your convenience, we offer appointment reminders via SMS or phone call. Please ensure that we have your up-to-date contact details on file.
- If you are unable to attend your appointment, please notify us as soon as possible following our cancellation policy.
Transfer of Medical Records
Transferring Records to Woodberry Family Practice:
- If you are a new patient and wish to transfer your medical records from another practice, please inform our reception staff. We will provide you with a consent form to authorize the transfer.
- Once we receive the completed consent form, we will request your records from your previous healthcare provider. Please allow up to 14 days for this process to be completed.
Transferring Records from Woodberry Family Practice:
- If you are moving to another practice and need your records transferred, you will need to complete a consent form authorizing the release of your records.
- A transfer fee of $40 may apply to cover administrative costs. This fee is payable before the transfer is initiated.
- Please allow up to 14 days for the transfer of records to be processed. We will ensure your records are securely transferred to your new healthcare provider.
Confidentiality
All patient records are treated with the utmost confidentiality. We comply with all relevant privacy laws to ensure your personal information is protected.
